Anti-Coagulant medicines such as warfarin can prevent life threatening conditions. However, they can be dangerous if people taking them are not monitored regularly and advised which dose to take. When a patient requests a prescription for anti-coagulation, the prescriber will check that the necessary monitoring has been carried out.
